Last recharge, Active plan details. Card type - Prepaid/Postpaid.
3) Email Address and Password.
4) Voter Identity Card (EPIC) Number or Voter ID details manually.
5) POI (Prof of Identity) as supporting document: PAN card number, Date of Birth, Father/Husband/any other name in mention PAN card.
